Yale Health provides a checklist of materials that ALL new students need to review and forms to submit in order to be cleared to register for courses for the the fall term.
-You must provide documents showing you have the state-required vaccinations
-You must submit a health history form
-You MUST submit these Yale HEALTH forms even if you plan to waive the Yale Health Hospitalization & Specialty coverage offered to you. These are Yale health and state requirements for all enrolled students.
You must provide these health forms by the deadline, in order to be cleared IN ADVANCE of your starting at the University at Orientation (or earlier if an official early start). Failure to submit complete forms will result in a “health hold” which will prevent you from getting your ID at New Student Orientation and may prevent you from registering for courses until it it resolved.
The vaccination requirement only covers the enrolled student, but spouses/partners and children are urged to have appropriate vaccinations, and to bring their health records with if enrolling in Yale Health.

Students who determine that they wish to waive the University’s coverage must do so online by September 15. Yale Health coverage begins for new GSAS students on August 18, which is the Friday before the start of Graduate School Orientation.
